Patient’s Charter

before admission

Administrative formalities are required in order to obtain either full or part payment from organizations (social security, mutual insurance companies, etc.) for the cost of your hospitalisation and in order to avoid any inconvenience over possible litigation or pointless payment of money.

Should your file be incomplete, please have the missing documents forwarded to us, either by mail, or by leaving them at the Admissions Office.

 


you have confirmed your admission following a pre-ANAESTHETIC consultation and have provided the administrative documents required:

ADMINISTRATIVE documents:
  In order to facilitate your admission, you must be in possession of your identity papers.

if you have general insurance or are affiliated to an agricultural insurance programme:
  Your registration card stating your benefits.

if you insured under social security:
  Your most recent registration for Social Security and your Vitale card.
Your certificate of registration from your supplementary fund.

If you make voluntary contributions:
 Your most recent receipts for contributions from URSSAF.

if you are retired:
 Your certificate of registration or Vitale card.
 Your pension status.

If you are the victim of an accident at work:
 Your registration card.
 An accident report (care given), supplied either by your employer or the CPAM.
 In all cases, the card or certificate from your supplementary fund.

items to bring with you

apart from administrative documents necessary for admission:

medical documents:
 Form supplied to you by your surgeon when you came for consultation.
 Specialist doctor’s letter if this has not already been submitted.
 Laboratory and X-ray results.
 Specialists’ report, electrocardiograms.
 Card denoting your blood group.
 Vaccination card for children.
 Prescription indicating current medication and treatment.
 All documents, even old ones which do not appear to be related to your current condition, may play an important role in prescribing your treatment

personal Effects:
 Clothes (pyjamas, nightgown, etc.).
 Toiletries (towel, facecloth, soap, razor, toothbrush etc.).
 Guaranteed cheque for supplementary expenses.

certificates for time off work and all medical certificates must be asked for from the secretariat for surgeons on admission:
 After formalities and admission examinations have been completed, you will be accompanied to, and settled into, your room.
 If you have come from another administrative area (département) to receive treatment in our establishment, there may be a difference in prices per day and for your operation.
 Before your hospitalisation, it would therefore be prudent to contact your Fund for Sick Insurance in order to obtain their agreement over payment.

In order to preserve your anonymity, please inform the admissions office or the supervisor for health care and the necessary precautions will be taken.

in case of urgent admission

 

If your condition requires your immediate admission for health care, formalities can be carried out by a member of your family, the ambulance crew, or later by the admissions office who will come to your bedside.

hospitalisation of a child

 

infants and children under 4:
 We recommend that at least one parent accompanies the child and brings his/her health card.

minors:
 Authorisation for the operation will be requested. This document is needed for admission should emergency surgical intervention be required.
